People move between pens and vials, and they carry vocabulary across that does not survive the trip. The most common casualty is the word "click".



A pen click is a device-specific increment. The pen holds a solution at a concentration set by the manufacturer, and the dial advances by a fixed amount of drug per click. The click therefore corresponds to a quantity of drug, and that quantity is a property of that pen model. Two pens from different manufacturers, or two strengths of the same pen, do not have the same click.



A syringe unit is a volume. One unit on a U-100 insulin syringe is one hundredth of a millilitre and always has been. It corresponds to a quantity of drug only once you also state the concentration of what is in the barrel.



So "I take twelve clicks" carries a dose if you know the pen. "I take twelve units" carries no dose at all unless you also say what the concentration was. Both statements feel equally specific to the person saying them, which is why forum advice moves between them so freely and why so much of it is unusable.



The practical consequence: never convert a click count directly to a unit count. Convert the click count to milligrams or micrograms using the pen's own specification, then convert that quantity to units using your own concentration. Two steps, both of which you can check, instead of one step that is a guess.



A worked example. Suppose a pen delivers 0.25 mg at a given dial setting and you are moving to a vial reconstituted to 2.5 mg/mL. One unit of your solution holds 0.025 mg. So 0.25 mg is 10 units. The click number and the unit number are unrelated integers that happen to describe the same dose.



When you record the switch, record both, once. Write the pen setting you were on, the milligram equivalent, and the new unit count, in the same entry on the day of the change. That one row is what makes the history before and after the switch comparable instead of two disconnected series.
